• Immunizations 2 months-18 years
  • Must have original shot record.  School health records are not accepted.
  • We provide immunizations to those who:
    • have medicaid
    • have no insurance
    • have insurance that does not cover immunizations
• Variety of Health Screenings
• Giveaways for all attendees!
• Free lunch for Children.

    • current check stubs
    • each child’s birth certificate and Social Security card
    • state-issued ID (drivers license, ID card, etc)
